You may read below the English translation of the Yiddish-language questionnaire that was sent out to all the prospective actors, actresses et al who were given the opportunity to have their biographical information appear in what would turn out to be seven volumes (one volume yet to be published) of the "Lexicon of the Yiddish Theatre".

1.) Exact date of your birth

2.) The place and the country

3.) Your correct name

4.) Your stage name

5.) Your lineage (pedigree), your family, what was relevant to the Yiddish theatre, what was their job?

6.) Where do you learn, where did you learn, and where did you complete your learning?

7.) How did you arrive on the stage; why, where and when was the first time you performed on the stage?

8.) Exact dates of your acting activities.

9.) Since today or since you were a director, a regisseur, have you your own theatre?

10.) Have you acted on non-Yiddish stages; if yes, when, where and in what?

11.) Do you also write, or do you translate plays? Have you published them, were they performed, where and when?

12.) Have you acted in film, and if yes, when, where and in what?

13.) Have your children or family been on the Yiddish or non-Yiddish stage?

14.) Have you ever published something about the theatre, such as memoirs, etc. -- if yes, when and where?

Comment along with your response, sending also your photograph, the photograph must be recent. If not, write the year when it was taken. Must contain your face, less any garments(?)

Yiddish actors! Strive to help yourselves by keeping strictly to the bare realities, because the work is historical and you carry alone the accountability for it, terms not with any reference. The best yikhus is yikhus itself; write one answer not to dry (?), and send it away to the following address:
